The services of the DAK doctors appointment hotline are provided by 4sigma GmbH on behalf of DAK-Gesundheit and are free of charge for you. On the following pages you will be asked to provide information about yourself. This includes: salutation, surname, first name, date of birth, insurance number, as well as your telephone number or email address for the answer, information about the desired specialist specialty and health data that is absolutely necessary to arrange an appointment, supplemented by information about the accessibility of the desired practice (e.g. distance from home, accessibility by public transport, accessibility).
This data will be used exclusively to provide the services of the DAK doctors appointment hotline and will be deleted after six months at the latest. DAK-Gesundheit only receives your insurance number and the number of arranged/not arranged appointments from this data for billing purposes.
Your data will of course be treated confidentially and will not be passed on to third parties except for making an appointment. Your consent is voluntary and you can do so at any time to 4sigma GmbH at dak-medizin-service@4sigma.de or at DAK- Health revoked. However, the services of the DAK doctors appointment hotline can no longer be used if you revoke your consent.
